macai is a legitimate AI client for macOS. Access to photos or files is normal when you choose to attach them to chats, and notifications are expected if you want message or update alerts. Your privacy also depends on the AI providers you connect, because prompts and attachments sent through those services are handled under each provider's policies. If you enable iCloud sync, your app data is also stored through your Apple account. Use only the providers and models you trust, and avoid sending sensitive information unless you are comfortable with where that data will go.
Recommendation: Keep macai if you use it to access AI services from your Mac or want one place to manage chats across providers. If you no longer use it, removing the app is reasonable, but you may want to export or back up conversations first if they matter to you.
